OK, having sorted out my ReWire problem by reinstalling ReWire from the Propellerheads site, I've now run a test with VST Shell and I'm happy to say it's all working fine here (even the arpeggiator sync works!).
 
It's been a while since I'd tried it, but basically I just followed my own instructions in the VST Shell ReadMe, ie:

"1. Launch Pro Tools
2. Launch VST Shell and click the Audio Driver button - select the ad_rewire driver, load your VSTi & switch DSP on.
3. In Pro Tools create a new (stereo) track and in the insert menu select max/msp. Audio from VST Shell should now appear on the new track, and  with sync enabled, the arpeggiator should slave to Pro Tools
4. To record the result to pro Tools whilst monitoring it, send the track to a bus, then create a new stereo track and select that bus as it's input. Bring the level up with the bus send, and record onto the new track."

As an addendum, I would say create the PT track with ReWire receiver /before/ turning on the DSP in VST Shell - it's no big deal, but you'll get an error message in the status window about the ReWire mixer not being on if you do it the other way round.
 Also, as I'm using PT 7.3 now, the track insert is found under 'Instruments' and of course it will say "VST Shell 3.6" , not "max/msp"

Of course we're only talking VST /instruments/ here, though you can use the Shell's built-in FX matrix to send the VSTi's through VST effects.

The test set-up I used is as follows: G5 Dual 2GHz running OS 10.4.11, the latest version of VST Shell (Jan 07), Max/MSP Runtime 4.6.3, Pluggo Jr. 3.6.1, Pro Tools LE 7.3.1cs6
